Nether wart is a bit complicated to get since you'll need to head into the Nether by making a portal. Nether wart spawns and is grown in Nether fortresses and bastions.

While you're at the Nether fortress, you'll want to defeat some blazes because they drop blaze rods. Placing some blaze rods on a crafting table will give you blaze powder.

Finally, you'll want to get your hands on a ghast tear, which is dropped by killing ghasts.

The ghast mob spawns in the Nether as well. Be careful though, they may be difficult to kill in a place where the ghast tears can drop into lava and be destroyed!
